Please add your return address, fax number or e-mail address (and telephone number for discussion of details). The answers are sent to the applicants in written form in statutory terms. Provision of personal data with identification is not possible.

Information on the Institute activity in accordance with Act no. 106/1999 Coll., on Free Access to Information, in wording of later amendments, can be provided in written or oral form at the same address or at the telephone number +420 224 972 243.

Regional Units of IHIS CR provide specific regional information.

If it is not possible to provide required data, the Institute issues a written resolution. It is possible to appeal against this resolution to Ministry of Health, which decides about the appeal in a procedure set by the Minister´s order.

Scale of fees for provided data

The scale of fees is available from the Heads of Sections and Departments (see Contacts).

The aim of this application is to provide topical information from the Registry of Health Establishments about the structure of the network of health care facilities and about the profile of health care offered in selected areas. It provides information on all state and non-state health establishments that obtained registration, and on their structure by activity branches. Information can be selected in term of the type of establishment, type of the founder and by health care branches. The application serves for getting up-to-date information about the changes in the structure of the network of health care facilities and it allows to find specific health establishments.
